Sorweide Family Medicine's office is warm and inviting with hot coffee always available.


Appointments:  We make every effort to provide medical assessment of all acute illnesses or problems on the day they are requested.  If you need to cancel your appointment, please call as soon as possible.  No show appointments are subject to a $25 fee.

Telephone Calls:  Our phones are answered daily during office hours. (closed noon to 1 for lunch)   If our lines are all busy, you may be directed to voicemail.  We check these regularly and will promptly return your phone message.  All emergencies, please dial 911.  If you need to speak to the on-call physician after hours,  you may dial 955-3893. Valley Immediate Care is available after hours for non-life threatening emergencies.

Prescription Refills:  Please call your pharmacy at least 48 hours before you need a prescription refilled.  This allows time for the pharmacy to contact our office for authorization. (prescriptions are not authorized after hours)

Medical Forms:  If you have a medical form that needs to be completed by Dr. Sorweide, please call to schedule an appointment.  Forms will NOT be completed without a one on one meeting with the doctor.

   Medical records can be obtained for $30 prepaid.  Records transferred to another physician are free. 

Referrals:  If you would like to be referred to a specialist, please set up an appointment with our receptionist.  Our referral coordinator will process your request.  Most referrals take 48 hours to process.

Insurance and Payments:  Please be familiar with the type of insurance you carry.  Bring your insurance card to your appointment and notify us if there are any changes.  All co-pays and deductibles must be paid at the time of service.  We accept cash, checks, debit, Visa or Mastercard only.
